Behavioral Health Care Coordinator

The Columbia County Department of Human Services/Mental Health Center is currently recruiting for a full time Adult Behavioral Health Care Coordinator (BHCC).   Permanent appointment to the position will be provisional pending the results of a civil service examination to be given on May 13, 2017.  The BHCC will provide comprehensive care management to adults diagnosed with behavioral health and/or chronic medical disorders who experience deficits in the areas of educational, social, vocational and interpersonal functioning. This is a position serving adults initially, but dependent upon operational needs, may potentially be serving children. The BHCC will assist clients in obtaining services and in developing supports through referral and linkage to community resources.  Minimum qualifications:  bachelor’s degree in social work, psychology, nursing, rehabilitation, education, occupational/physical/recreation therapy, counseling, community mental health, child and family studies, or sociology  AND  four (4) years of full-time experience in a behavioral health setting providing direct services to mentally ill and/ or chemically dependent consumers; or experience in linking mentally ill and/or chemically dependent consumers to a range of services essential to successful living in the community setting.  A master’s degree in one of the fields listed above may be substituted for two years of experience.  Possession of a valid NYS Driver License is required at the time of appointment and beyond.

Twin County Recovery Services, Inc.- Behavioral Health Care Coordinator

Twin County Recovery Services, Inc. is currently recruiting for a full time Adult Behavioral Health Care Coordinator (BHCC).  This is not a Civil Service position, but offers employment through a not for profit agency with grant funding and oversight by Columbia County Department of Human Services. The BHCC will provide comprehensive care coordination services to adults diagnosed with substance use disorders, mental health disorders and/or chronic medical conditions.  The BHCC will assist clients in developing supports through referral and linkage to community resources and coordinate/monitor indicated services.  Minimum qualifications:  bachelor’s degree in one of the following areas: social work, psychology, nursing, rehabilitation, counseling, community mental health, and two (2) years of full-time experience in a behavioral health setting providing direct services to people with substance use disorders and/or mental health disorders, or a master’s degree in one of the fields listed above with one year of experience. Possession of a valid NYS Driver License is required at the time of appointment and beyond.
