Regional Planning Consortium (RPC) Creates Linkages Among Three Capital Region Health Homes Serving Adults

To help increase knowledge and awareness of the options available for its HARP-enrolled members, local Health Home, Capital Region Health Connections (CRHC), hosted a panel discussion and networking session earlier in the year for its staff, which was entitled "Navigating HCBS for HARP Members."

The Capital Region RPC, after receiving positive feedback from attendees who also serve on the RPC's HARP/HCBS/Health Home work group, recommended to CRHC that this event be replicated and expanded to all Health Home care coordinators in the region. 

In response to this recommendation, on December 12th CRHC collaborated with Adirondack Health Institute (AHI) and Hudson River Healthcare to provide a regional event inviting Health Home care coordinators from each county in the Capital Region (Albany, Columbia, Greene, Saratoga, Schenectady, Rensselaer). 

At this event, designated HCBS providers in the community will share their knowledge of available resources and the specific services their agency provides to HARP Members.
